How to start?

1) filling in the form

PIt is possible to download the form and print it out (PDF file) (it is necessary to have one double-sided sheet) or you are free to come during office hours to the seat of Modus driving school.

2) medical check

Filed form needs to contain a confirmation of medical fitnessti (your general practicioner confirms that). As well he/she confirms Report on Medical fitness to Drive Motor Vehicles (PDF file). You can download this document here or feel free to colect it in the seat of MODUS driving school. 

3) payment

• in cash in the seat of MODUS driving school when handling over the filled form
• pay in slip
• bank transfer
